2019: A Year of Innovative Impact at HealthFinders

2019 was an exciting and important year in the life of HealthFinders Collaborative (HFC). We are positioning our organization to double its impact within the community by growing our capacity to serve the community’s needs and working every day to ensure that underserved neighbors are receiving qualify, cost-effective healthcare.

Generated Community Impact An independent study finds that every $1 invested in HealthFinders generates over $16 in community impact.

Served as a Missing Link

Mayo Clinic Health System Faribault piloted a partnership with HealthFinders to improve the health outcomes of their Somali obstetrics patients in the Faribault area. Of the unique obstetrics care collaboration, Dr. Kristina Rauenhorst shared, “Our HealthFinders community health worker has played an active role in the care of Somali obstetrics patients and has been invaluable in improving the care, understanding, and trust of this group of patients in the Faribault community. She was the link that had been missing in all of our prior efforts to improve care for these patients in our community.”


Provided High-Quality Care Minnesota Department of Health certified HealthFinders Collaborative as a Certified Health Care Home.

Set the Stage with a Successful Gala HealthFinders’ annual spring gala revenue exceeded $100,000 for the first time. Save the date for the 2020 gala – Saturday, March 21! At the gala, Joe Lally of Delta Dental of Minnesota Foundation announced the organization’s lead gift of $1,000,000 for the HealthFinders combined campaign to help expand dental services. This announcement brought an audible gasp and rousing applause from guests.

Invested in the Future

On May 1, 2019,

HealthFinders purchased a building located at 1415 Town Square Lane in Faribault. The nearly 9,000-square-foot building will serve as a holistic wellness hub for HealthFinders’ services in Faribault, giving us room to grow and the ability to operate many of our programs under one roof – from medical, dental, and mental health services to group fitness, advocacy, and nutrition demonstrations!

Promoted Intercultural Respect HealthFinders began offering Intercultural Effectiveness Seminars to groups in Rice County in an effort to promote intercultural respect and understanding, which received rave reviews. Expanded Our Services and Took Care Beyond Clinic Walls

HealthFinders’ dental team went into the community,

bringing dental services to hundreds of local elementary students. Offering these services to un-insured and under-insured students in the school both ensures the students receive the dental care they need and minimizes the time missed from school to receive dental care. Beginning this fall, Emily Carroll, HFC’s director of clinical care and certified nurse practitioner (CNP), started holding “teen clinics” in Faribault High School during school hours. Teens can access sexual health services, STI testing, and general healthcare.

Convened the Community When HealthFinders convened the initial Local Health System Summit over a year ago, we expected the event to be annual or quarterly at best. The group has continued to meet monthly, with local leadership from Allina Health, Mayo Clinic Health System, Northfield Hospital + Clinics, Sterling Pharmacy, Rice County Public Health, and HealthFinders. The organizations collaborate to improve the healthcare of underserved neighbors in our community. Charlie Mandile, HFC’s executive director, serves as the meeting convener and facilitator.

Improved Health Outcomes and Cost Savings Results show that HFC’s new in-house point-of-care testing lab resulted in patients saving time and showing improved diabetes and heart disease outcomes, with an estimated cost savings of 50%. Through a global, five-year HealthRise partnership with Medtronic Foundation, HealthFinders’ diabetic patients who achieved control of their diabetes increased by 16% over 2018.
