Community Health Workers

Community Health Workers (CHWs) are frontline agents of change, helping to reduce health disparities in underserved communities.

 

At HealthFinders, CHWs offer access to services, provide health education, support care delivery, and promote advocacy. While delivering a culturally appropriate health education and information, our CHWs also help people get the care they need, give informal counseling and guidance on health behaviors, advocate for individual and community health needs, and provide some direct services such as first aid, blood pressure, and glucose level screening. What is particularly unique in this model is the fact that our CHWs meet our clients in their own environment such as where they live, eat, play, work, and worship.

Maternal Health Program

Mayo Clinic Health System and HealthFinders Collaborative have teamed up to provide Community Health Workers (CHWs) as a resource for expecting parents. CHWs work to bridge the patient to their health care system by providing patient advocacy, support, and ensure cultural beliefs are addressed during health care visits.

CHWs can assist with the following:

  • Bridged care with the care team

  • Patient centered & culturally appropriate health visits

  • WIC & SNAP benefits information

  • Housing resources

  • MNSure & Medical Assistance enrollment

  • Food security access

  • Assistance scheduling transportation needs

  • Home visits to address chronic disease management

  • education and support regarding family planning

Meet our Community Health Workers

LAURA SAN MIGUEL

Laura is a Community Health Worker at HealthFinders Collaborative and she thinks maternal health is very important to be well informed about prenatal, postnatal and baby care. As a mother herself, Laura recognizes and acknowledges the nuances of maternal health has within the healthcare system.

 

MARYAN HASSAN 

Maryan is a Community Health Worker at HealthFinders Collaborative and works with the Maternal health providers at Mayo Clinic Health Sytem. Her goal at HealthFinders is to help her community grow and to make patients feel at home, just like she did when she joined the HealthFinders team.
