HealthFinders Collaborative offers primary care services across two clinic locations in Faribault and Northfield. Our clinics offer a range of care, including:

• Physical examinations

• Acute illness care

• Disease management (diabetes, blood pressure, and others)

• Disease prevention and wellness

• Reproductive Women’s health in collaboration with The Center Clinic

• Dental care

• Medication assistance

• MNsure and other Advocacy Services

In addition to clinic staff, both locations are supported by volunteer providers, nurses, interpreters, receptionists, and student volunteers. We offer some lab services in house with additional lab work and diagnostic imaging provided by partners like Allina, Mayo, and Northfield Hospital & Clinics.

Medication Assistance

Through the Medication Assistance Program we are able to offer a reduced co-payment for on-formulary medications ($4 for a 30-day supply, $9 for a 90-day supply to any uninsured Rice County resident. For some patients we are able to provide waivers for these co-pays.   This program works in close collaboration with our local partner Sterling Pharmacy to offer prescriptions not only to HealthFinders patients, but also patients in local emergency rooms and clinics without coverage for medications. Sterling Pharmacy provides prescription delivery in both Northfield and Faribault, automatic refills, and other prescription services. Our unique community collaboration with Sterling Pharmacy allows us to better meet the needs of patients, adding a community pharmacist to our care team.

MNsure Navigation

We have MNsure-approved navigators who can assist with insurance enrollment and other MNsure enrollment issues. Our efforts include helping patients apply for health insurance, assisting patients through bill-pay or patient-assistance programs, and connecting patients to other community organizations and social service programs throughout Rice County. We gear our assistance to the specific needs of each patient and help contact the appropriate agencies to ensure that applications and enrollment are completed.
