Insurance Advocacy and Social Services Support

Advocating for Patient Health and Wellness

At HealthFinders Collaborative, we advocate for the health and wellness of our patients. We provide assistance based on each patient’s specific needs, while connecting them to community resources and social service programs. We also help patients enroll in health insurance through MNsure. HFC has several bilingual MNSure-approved navigators to help you get connected to care year-round – during open enrollment, or if you have a life circumstance that may make you eligible – our team offers year-round assistance at all locations.


Food, Housing, and Social Services Support

In addition to MNsure, our advocates assist patients with other services supportive to their overall health, including food and housing support and social services. If HealthFinders advocates aren’t able to assist, they work with partners to connect patients to the services they need.

Community Engagement

We engage communities through conversations and building authentic long-term relationships that:

  • surface individual and community concerns, passions, strengths

  • cultivate space, support individual strengths, and learning

  • listen with our heart, knowing listening and talking as support, show compassion

  • build trust first

  • engage social barriers

  • build on open-ended conversations

  • we aren’t always going to have the answers


With our findings we aim to Promote health and remove barriers by connecting community engagement to existing teams and programs within HFC, convene conversations with outside partners as appropriate, engage the HFC board of directors in our work.

  • Supporting other programs to align with community needs

  • Develop unique programs in response to findings

  • Cultivate and support community leaders to engage in our work

  • Giving platform to extend voices to broader community


We will remain accountable to each other and the community by incorporating measurement and surveys where feasible, tracking progress on current initiatives, and always following up with community.

Schedule a MNSure Appointment

Our MNsure-approved navigators help with enrollment and other MNsure procedures. As Minnesota Healthcare Application Agents, we help patients who qualify get enrolled in coverage programs through our Minnesota Department of Health certification. Our efforts include helping patients apply and offering patient-assistance programs.
